This is a combination drug consisting of bupivacaine, an amide-type local anesthetic, and dexamethasone sodium phosphate, a synthetic glucocorticoid corticosteroid. Bupivacaine acts by blocking voltage-gated sodium channels in neuronal membranes, thereby inhibiting nerve impulse conduction and producing local or regional anesthesia or analgesia. Dexamethasone sodium phosphate provides potent anti-inflammatory effects by modulating immune responses and suppressing inflammation through glucocorticoid receptor activation. When used together (often as an admixture for injection), dexamethasone serves as an adjuvant to prolong the duration of analgesia provided by bupivacaine in various regional anesthesia techniques such as peripheral nerve blocks and epidural blocks[1][2][8]. The combination is primarily indicated for surgical anesthesia/analgesia where prolonged pain control is desired.
