bupivacaine + epinephrine

**Bupivacaine + epinephrine** is a fixed-dose injectable combination of the long-acting amide local anesthetic bupivacaine hydrochloride and the vasoconstrictor epinephrine, commonly supplied with epinephrine 1:200,000. Bupivacaine reversibly inhibits voltage-gated sodium channels in peripheral nerves, preventing initiation and propagation of nerve impulses. Epinephrine activates adrenergic receptors, with local alpha-1-mediated vasoconstriction reducing local blood flow, slowing systemic absorption of bupivacaine, prolonging anesthesia, and reducing peak systemic exposure. It is used in adults to produce local or regional anesthesia and analgesia for surgical, dental, diagnostic, therapeutic, and obstetrical procedures through infiltration, peripheral nerve block, caudal block, and epidural block.
