Overview

bupivacaine + lidocaine + epinephrine + sodium bicarbonate is a multi-component local anesthetic solution being evaluated by the Hospital Italiano de Buenos Aires for the management of postoperative pain following carpal tunnel release surgery. This combination therapy utilizes bupivacaine and lidocaine, both amide-type local anesthetics that block voltage-gated sodium channels to prevent the conduction of nerve impulses. Lidocaine provides a rapid onset of action, while bupivacaine offers a longer duration of analgesia. Epinephrine is included as a vasoconstrictor to prolong the local effect and reduce systemic toxicity by slowing absorption. Sodium bicarbonate is added as a buffering agent to adjust the pH of the solution, which can decrease the pain of injection and potentially speed the onset of the anesthetic block.
