Bupivacaine + methylene blue is a combination of two drugs used primarily for local or regional anesthesia and extended postoperative analgesia. Bupivacaine is an amide-type local anesthetic that works by blocking voltage-gated sodium channels in neuronal membranes, thereby inhibiting nerve impulse conduction and providing anesthesia and analgesia. Methylene blue, traditionally known as a dye and treatment for methemoglobinemia, has been shown to enhance the duration of analgesia when combined with local anesthetics. Its mechanisms include anti-inflammatory effects, inhibition of oxidative stress, reversible demyelination of nerves at low concentrations (which prolongs pain relief), blockade of sodium channels, inhibition of soluble guanylate cyclase and nitric oxide synthase pathways involved in pain signaling. The combination has been studied for periarticular infiltration after surgery (such as knee arthroplasty) and epidural anesthesia in both human and veterinary settings; it provides synergistic enhancement of analgesic effect with minimal reported toxicity at therapeutic doses[1][3][4].
