Overview

bupivacaine + morphine is a combination of the amide-type local anesthetic bupivacaine and the opioid analgesic morphine, utilized in a clinical trial sponsored by the University Hospital of North Norway. Administered intrathecally as an adjunct to general anesthesia, this combination is intended to manage postoperative pain and reduce systemic opioid requirements in patients undergoing laparoscopic abdominoperineal rectal amputation. Bupivacaine acts by binding to the intracellular portion of voltage-gated sodium channels, thereby blocking sodium influx and inhibiting the initiation and conduction of nerve impulses. Morphine serves as an agonist at mu-opioid receptors in the spinal cord, modulating pain transmission in the dorsal horn. This dual-action approach provides regional analgesia that targets both peripheral nerve conduction and central pain processing.
