Overview

This multimodal analgesic regimen, utilized by Meir Medical Center, is designed for postoperative pain management in patients undergoing total knee arthroplasty. The protocol involves a spinal block consisting of intrathecal heavy bupivacaine (Marcaine) and morphine, combined with a high-volume local anesthetic infiltration (LIA) of bupivacaine and adrenaline (epinephrine). Bupivacaine provides regional anesthesia by blocking voltage-gated sodium channels, preventing the conduction of pain signals. Morphine acts as an agonist at mu-opioid receptors in the spinal cord to provide extended analgesia. Epinephrine is included as a vasoconstrictor to prolong the local anesthetic effect and minimize systemic toxicity. This combination is being evaluated in Phase 4 clinical trials to compare its efficacy in pain reduction and physical therapy outcomes against general anesthesia.
