Busulfan + bortezomib is a combination conditioning regimen investigated for use in autologous hematopoietic stem cell transplantation (AHSCT), particularly for patients with multiple myeloma. This regimen combines busulfan, a bifunctional DNA alkylating agent that causes interstrand cross-linking and subsequent cell death, with bortezomib, a first-in-class proteasome inhibitor. Bortezomib reversibly inhibits the 26S proteasome, disrupting protein degradation pathways, which leads to the accumulation of pro-apoptotic proteins and sensitizes malignant plasma cells to the cytotoxic effects of alkylating agents. In clinical research, this combination is frequently evaluated as part of the BUMELVEL regimen, which also includes melphalan, to enhance the depth of response and improve progression-free survival in transplant-eligible myeloma patients.
