Overview

This is a multi-agent combination regimen consisting of five drugs—busulfan, fludarabine, cyclophosphamide, antithymocyte globulin (ATG), and cyclosporine—used primarily as a conditioning and immunosuppressive protocol for patients undergoing allogeneic hematopoietic stem cell transplantation (HSCT). The regimen combines alkylating agents (busulfan and cyclophosphamide) with a purine analog (fludarabine) to achieve myeloablation or reduced-intensity conditioning. ATG is included for T-cell depletion to reduce the risk of graft-versus-host disease (GVHD), while cyclosporine acts as an immunosuppressant to further prevent GVHD by inhibiting T-lymphocyte activation. This combination is used in various settings including acute myeloid leukemia in remission and severe aplastic anemia when transplantation or intensive therapy is indicated[1][2][3][4][5].
