This combination therapy is an investigational second-line treatment regimen for patients with microsatellite stable (MSS) metastatic colorectal cancer (mCRC). The regimen centers on cadonilimab (AK104), a humanized tetravalent bispecific antibody developed by Akeso Biopharma that simultaneously targets the PD-1 and CTLA-4 immune checkpoint pathways to reverse T cell exhaustion. This immunotherapy is combined with the FOLFIRI chemotherapy backbone (5-fluorouracil, leucovorin, and irinotecan) and a targeted monoclonal antibody selected based on the patient's RAS mutation status: cetuximab (anti-EGFR) for RAS wild-type tumors or bevacizumab (anti-VEGF) for RAS mutated tumors. The regimen is currently being evaluated in a Phase 2 clinical trial sponsored by the Second Affiliated Hospital, School of Medicine, Zhejiang University, to determine if dual checkpoint blockade can enhance the efficacy of standard-of-care chemotherapy and targeted therapy in the typically immunotherapy-resistant MSS colorectal cancer population.
