Overview

This is a combination regimen consisting of four agents: - **Cadonilimab** is a tetrameric bispecific monoclonal antibody targeting PD-1 and CTLA-4 immune checkpoints. It acts as an immune checkpoint inhibitor to enhance antitumor immunity. - **LM-302** (also known as TPX-4589) is an antibody-drug conjugate (ADC) that targets Claudin 18.2 (CLDN18.2), a tight junction protein overexpressed in certain solid tumors such as gastric and biliary tract cancers. The ADC delivers the cytotoxic payload monomethyl auristatin E (MMAE) to CLDN18.2-positive tumor cells. - **S-1** is an oral fluoropyrimidine-based chemotherapy composed of tegafur (a prodrug of 5-fluorouracil), gimeracil, and oteracil potassium; it inhibits DNA synthesis in rapidly dividing cancer cells. - **Paclitaxel** is a microtubule-stabilizing chemotherapeutic agent that disrupts mitosis by preventing microtubule disassembly, leading to cell cycle arrest and apoptosis. This combination regimen has been investigated for the treatment of CLDN18.2-positive advanced gastric cancer and other solid tumors[3][5]. The rationale for combining these agents includes synergistic effects between immunotherapy (cadonilimab), targeted therapy/ADC (LM‑302), cytotoxic chemotherapy (S‑1), and microtubule inhibition (paclitaxel).
