Cadonilimab + nab-paclitaxel is a combination therapy comprising cadonilimab (AK104), a first-in-class bispecific antibody targeting programmed cell death protein 1 (PD-1) and cytotoxic T-lymphocyte-associated protein 4 (CTLA-4), and nab-paclitaxel, an albumin-bound formulation of the microtubule inhibitor paclitaxel. This combination is being investigated as a second-line treatment for patients with advanced gastric adenocarcinoma or esophagogastric junction (EGJ) adenocarcinoma who have progressed on prior PD-(L)1 inhibitor therapy. Cadonilimab works by simultaneously blocking two inhibitory immune checkpoint pathways to restore anti-tumor T-cell activity, potentially overcoming resistance to single-agent PD-1 blockade. Nab-paclitaxel provides cytotoxic activity by stabilizing microtubules and inducing cell cycle arrest. The combination is currently being evaluated in a Phase II clinical trial sponsored by China Medical University.
