Cadonilimab + surufatinib is a combination therapy comprising a bispecific antibody and a small molecule multi-kinase inhibitor. Cadonilimab (AK104) is a first-in-class humanized tetravalent bispecific antibody targeting both programmed cell death protein 1 (PD-1) and cytotoxic T-lymphocyte-associated protein 4 (CTLA-4). It is designed to achieve higher avidity in the tumor microenvironment where both targets are co-expressed, potentially improving efficacy and safety compared to the combination of separate PD-1 and CTLA-4 inhibitors. Surufatinib (HMPL-012) is an oral small molecule inhibitor of vascular endothelial growth factor receptors (VEGFR1, 2, and 3), fibroblast growth factor receptor 1 (FGFR1), and colony-stimulating factor 1 receptor (CSF-1R). While VEGFR and FGFR inhibition targets tumor angiogenesis, CSF-1R inhibition aims to reduce tumor-associated macrophages, thereby promoting an anti-tumor immune response. The combination is being evaluated for its synergistic potential in treating advanced malignancies, including small cell lung cancer (SCLC), by simultaneously targeting immune checkpoints, angiogenesis, and the myeloid-derived immunosuppressive microenvironment.
