Cadonilimab + TKI is a combination therapy being investigated for the neoadjuvant treatment of urinary system tumors, specifically urothelial carcinoma and renal cell carcinoma. Cadonilimab (AK104) is a first-in-class humanized tetravalent bispecific antibody that simultaneously targets the programmed cell death protein 1 (PD-1) and cytotoxic T-lymphocyte-associated protein 4 (CTLA-4) immune checkpoints. By blocking both pathways, it aims to enhance anti-tumor immune responses. In this combination, it is paired with a tyrosine kinase inhibitor (TKI), such as sunitinib or pazopanib, which inhibits vascular endothelial growth factor (VEGF) signaling and other pathways involved in tumor angiogenesis and proliferation. This combination is being evaluated by Tongji Hospital in a Phase II trial to assess its efficacy in achieving organ preservation (bladder or kidney) by reducing tumor burden before surgery.
