Overview

Calcium carbonate (13C) is a diagnostic agent labeled with the stable isotope carbon-13, primarily developed for use in non-invasive breath tests to assess gastric acidity. When ingested, the compound reacts with hydrochloric acid (gastric acid) in the stomach to produce calcium chloride, water, and 13C-labeled carbon dioxide (13CO2). The 13CO2 is absorbed into the systemic circulation and eliminated via the lungs. By measuring the ratio of 13CO2 to 12CO2 in exhaled breath using infrared spectroscopy or mass spectrometry, clinicians can estimate gastric pH and acid secretion levels. This methodology is particularly useful for evaluating the efficacy of acid-suppressing therapies, such as proton pump inhibitors (PPIs), and for diagnosing PPI-resistant gastroesophageal reflux disease (GERD).
