Overview

This refers to a combination therapy or fixed-dose combination of two classes of antihypertensive drugs: a calcium channel blocker (CCB) and a beta adrenergic blocker (beta blocker). These combinations are used when monotherapy with either agent is insufficient for controlling blood pressure, angina, or other cardiovascular conditions. Calcium channel blockers inhibit L-type voltage-gated calcium channels in cardiac and smooth muscle cells, leading to vasodilation, reduced cardiac contractility, decreased heart rate, and lower blood pressure. Beta blockers antagonize beta-adrenergic receptors in the sympathetic nervous system, reducing heart rate, myocardial contractility, systolic pressure, and overall cardiac output. The combined effect results in additive reductions in myocardial oxygen demand by lowering both heart rate and blood pressure more than either drug alone[1][2][5]. This approach can improve efficacy but requires careful patient selection due to the risk of additive adverse effects such as bradycardia or atrioventricular block[2].
