Calcium hydroxide + sodium hypochlorite is a compounded intracanal medication combination used in endodontic dentistry, specifically for root canal retreatment. It combines the long-term antimicrobial and anti-inflammatory properties of calcium hydroxide with the rapid tissue-dissolving and potent antimicrobial effects of sodium hypochlorite. Calcium hydroxide provides a highly alkaline environment (pH ~12.5) that denatures bacterial proteins and DNA, inhibits bacterial survival, and promotes periapical healing. Sodium hypochlorite serves as both a vehicle and a synergistic disinfectant, facilitating the dissolution of organic debris and rapid disinfection. This combination is primarily investigated for its ability to reduce periapical inflammation (measured by biomarkers like IL-1B) and microbial load in cases of periapical abscess or lesions during two-visit endodontic protocols.
