This combination therapy integrates the anti-PD-1 monoclonal antibody camrelizumab, the VEGFR-2 tyrosine kinase inhibitor apatinib, and the FOLFOX chemotherapy regimen, which consists of oxaliplatin, fluorouracil, and folinic acid. It is primarily being investigated for the treatment of advanced hepatocellular carcinoma (HCC). The rationale behind this combination is to leverage the synergistic effects of immune checkpoint inhibition, anti-angiogenesis, and cytotoxic chemotherapy. Camrelizumab restores T-cell activity against tumor cells by blocking the PD-1/PD-L1 pathway, while apatinib inhibits tumor vascularization and may improve the delivery of chemotherapy and the infiltration of immune cells into the tumor microenvironment. The FOLFOX component provides direct cytotoxic effects by inducing DNA damage and inhibiting pyrimidine synthesis. This multi-pronged approach aims to overcome the complex resistance mechanisms inherent in HCC and is currently being evaluated in clinical trials sponsored by institutions such as The First Hospital of China Medical University.
