Camrelizumab + apatinib + mitotane is a combination therapy being investigated as a first-line treatment for unresectable advanced adrenocortical carcinoma (ACC). This regimen combines three distinct therapeutic agents: camrelizumab, a humanized monoclonal antibody targeting the programmed cell death protein 1 (PD-1) receptor to enhance anti-tumor immune responses; apatinib, a small molecule tyrosine kinase inhibitor that selectively targets vascular endothelial growth factor receptor 2 (VEGFR2) to inhibit angiogenesis; and mitotane, an adrenolytic agent that inhibits steroidogenesis and induces mitochondrial damage in adrenocortical cells. This investigator-initiated study, led by West China Hospital, aims to improve outcomes in ACC, a rare and aggressive malignancy where standard treatments like the EDP-M regimen (etoposide, doxorubicin, cisplatin, and mitotane) show limited long-term efficacy. Preliminary results from the Phase II trial have shown promising objective response rates and disease control in this patient population.
