This combination therapy consists of **camrelizumab** (also known as carelizumab), a humanized monoclonal antibody targeting the PD-1 receptor; **irinotecan**, a cytotoxic chemotherapy agent; and **apatinib**, a small molecule tyrosine kinase inhibitor targeting VEGFR-2. The regimen is being investigated as a second-line treatment for patients with locally advanced unresectable, recurrent, or metastatic adenocarcinoma of the stomach and gastroesophageal junction (GEJ). Camrelizumab functions as an immune checkpoint inhibitor, blocking the interaction between PD-1 and its ligands (PD-L1 and PD-L2) to enhance T-cell-mediated anti-tumor immune responses. Irinotecan provides cytotoxic activity by inhibiting DNA topoisomerase I, leading to double-strand DNA breaks and cell death. Apatinib complements these effects by inhibiting tumor angiogenesis through the selective blockade of vascular endothelial growth factor receptor-2 (VEGFR-2). This triplet combination is currently being evaluated in a Phase II clinical trial sponsored by Nanfang Hospital, Southern Medical University.
