Camrelizumab + oxaliplatin + tegafur + oxaliplatin + capecitabine is a combination neoadjuvant chemo-immunotherapy regimen primarily investigated for the treatment of locally advanced gastric and gastroesophageal junction (GEJ) cancers. The regimen consists of camrelizumab (AiRuiKa), a humanized IgG4 monoclonal antibody targeting the programmed cell death protein 1 (PD-1) receptor, combined with either the SOX (oxaliplatin and tegafur/S-1) or XELOX (oxaliplatin and capecitabine) chemotherapy doublets. Camrelizumab functions as a checkpoint inhibitor, preventing the interaction between PD-1 and its ligands (PD-L1/PD-L2) to restore T-cell mediated immune responses against tumor cells. The chemotherapy components provide cytotoxic effects: oxaliplatin induces DNA damage through the formation of platinum-DNA adducts, while tegafur and capecitabine are prodrugs of 5-fluorouracil (5-FU) that inhibit thymidylate synthase, thereby impairing DNA synthesis. This combination is currently being evaluated in Phase 2 clinical trials (notably NCT05545436) to improve pathological response rates and surgical outcomes in patients with resectable gastric malignancies.
