Canakinumab + methotrexate is a combination therapy involving canakinumab, a fully human monoclonal antibody targeting interleukin-1 beta (IL-1β), and methotrexate, a folate antimetabolite and immunosuppressant. Canakinumab neutralizes IL-1β, thereby reducing inflammation in autoinflammatory and autoimmune diseases. Methotrexate inhibits enzymes involved in nucleotide synthesis, leading to anti-inflammatory effects through increased extracellular adenosine. This combination has been studied primarily for the treatment of rheumatoid arthritis (RA) in patients with inadequate response to methotrexate alone. Clinical trials have shown that adding canakinumab to stable doses of methotrexate improves therapeutic responses in active RA without significant new safety concerns[3][1]. The combination may also be used off-label or investigated for other inflammatory conditions.
