Candesartan + chlorthalidone is an oral fixed-dose antihypertensive combination developed by EMS for essential arterial hypertension. Candesartan is administered clinically as the prodrug candesartan cilexetil and is converted to candesartan, a selective angiotensin II type 1 receptor antagonist that reduces vasoconstrictive and aldosterone-mediated effects of angiotensin II. Chlorthalidone is a thiazide-like diuretic that inhibits sodium-chloride reabsorption in the distal convoluted tubule, promoting natriuresis and lowering blood pressure. Phase 3 studies evaluated fixed-dose candesartan cilexetil + chlorthalidone combinations, including 8 mg or 16 mg candesartan cilexetil with 12.5 mg or 25 mg chlorthalidone, for essential hypertension; registered development studies were withdrawn.
