Candesartan + cilnidipine is a fixed-dose combination (FDC) medication primarily indicated for the treatment of essential hypertension. It combines candesartan cilexetil, a long-acting angiotensin II receptor blocker (ARB), with cilnidipine, a unique calcium channel blocker (CCB). Candesartan acts by selectively antagonizing the AT1 receptor, preventing the vasoconstrictive and aldosterone-secreting effects of angiotensin II. Cilnidipine distinguishes itself from conventional CCBs by providing dual blockade of both L-type calcium channels (located in vascular smooth muscle) and N-type calcium channels (located in sympathetic nerve endings). This dual mechanism allows for effective vasodilation while simultaneously suppressing sympathetic overactivity, which helps prevent reflex tachycardia and provides superior renoprotective effects compared to L-type-only CCBs. The combination is widely utilized in Asian markets, particularly Japan and India, for patients whose blood pressure is not adequately controlled by monotherapy.
