Candonilimab + all-trans retinoic acid is an experimental combination therapy consisting of candonilimab, a tetravalent bispecific monoclonal antibody targeting both PD-1 and CTLA-4 immune checkpoints, and all-trans retinoic acid (ATRA), a small molecule derivative of vitamin A. Candonilimab works by simultaneously blocking the PD-1 and CTLA-4 pathways on T cells, thereby enhancing anti-tumor immune responses while reducing toxicity compared to traditional dual checkpoint blockade[1][5][6]. All-trans retinoic acid induces differentiation in certain cancer cells (notably in acute promyelocytic leukemia) and modulates gene expression through activation of nuclear retinoic acid receptors. The combination is being explored for synergistic effects in cancer immunotherapy, leveraging both enhanced T cell activation from checkpoint inhibition and potential tumor cell differentiation or immunomodulation from ATRA.
