**Cangrelor + prasugrel** refers to the concomitant or sequential use of two antiplatelet agents that both target the P2Y12 receptor on platelets but through different pharmacological profiles. **Cangrelor** is an intravenous, direct-acting, reversible P2Y12 inhibitor used for short-term platelet inhibition during percutaneous coronary intervention (PCI), providing rapid and potent platelet inhibition with quick reversibility upon discontinuation. **Prasugrel** is an oral thienopyridine prodrug that irreversibly inhibits the same receptor via its active metabolite, used for prevention of thrombotic cardiovascular events in acute coronary syndromes and during PCI. Current evidence indicates a pharmacodynamic drug-drug interaction when these agents are administered concomitantly, resulting in a temporary gap in platelet inhibition after cangrelor discontinuation due to insufficient availability of prasugrel active metabolite to bind P2Y12 receptors vacated by cangrelor. The combination is not indicated for routine clinical use, and guidelines recommend prasugrel be administered at the end of cangrelor infusion rather than concurrently[1][3][4].
