Cannabidiol + methylprednisolone is a combination therapy being investigated for the treatment of grade I/II acute graft versus host disease (GVHD). This therapeutic approach, studied by the Rabin Medical Center, combines the non-psychoactive phytocannabinoid cannabidiol (CBD) with the synthetic corticosteroid methylprednisolone. Cannabidiol is known for its potent anti-inflammatory and immunosuppressive properties, which it exerts through a complex mechanism involving the modulation of cannabinoid receptors (CB1 and CB2), G protein-coupled receptors (such as GPR55), and transient receptor potential (TRP) channels. Methylprednisolone serves as a standard-of-care systemic anti-inflammatory agent that binds to the glucocorticoid receptor to suppress the immune response. The combination aims to enhance the efficacy of GVHD treatment while potentially allowing for steroid-sparing effects in patients following allogeneic hematopoietic stem cell transplantation.
