CAPA + ivermectin refers to an experimental enhancement of the Capacitation Culture (CAPA) followed by In Vitro Maturation (IVM) protocol, specifically developed and studied at Mỹ Đức Hospital for treating infertility associated with Polycystic Ovary Syndrome (PCOS) or Polycystic Ovarian Morphology (PCOM). The CAPA phase is a pre-maturation step that utilizes C-type Natriuretic Peptide (CNP) to maintain oocytes in meiotic arrest, mimicking the natural follicular environment and allowing for cytoplasmic maturation. Ivermectin is added to this phase as a positive allosteric modulator of the P2X4 receptor, which is expressed in human granulosa cells and oocytes. This addition is designed to synergize with CNP-mediated signaling through the Natriuretic Peptide Receptor 2 (NPR2), thereby improving the synchronization of nuclear and cytoplasmic maturation. This optimized protocol aims to increase the yield of competent mature oocytes and improve clinical outcomes in IVM cycles, which offer a lower-cost and lower-risk alternative to conventional IVF by reducing the need for high-dose gonadotropin stimulation.
