This is a combination regimen consisting of three drugs: - **Capecitabine** is an oral prodrug of 5-fluorouracil (5-FU), a pyrimidine analog that inhibits DNA synthesis by interfering with thymidylate synthase, leading to cytotoxicity in rapidly dividing cells. - **Oxaliplatin** is a platinum-based chemotherapeutic agent that forms DNA crosslinks and inhibits DNA replication and transcription, resulting in cell death. - **Envafolimab** is a humanized single-domain antibody (nanobody) targeting programmed death-ligand 1 (PD-L1), functioning as an immune checkpoint inhibitor to enhance anti-tumor immune responses. The combination of capecitabine and oxaliplatin (often called XELOX or CAPOX) is widely used for the treatment of colorectal, gastric, and other gastrointestinal cancers[1][2][3][4][7]. Envafolimab adds immunotherapy via PD-L1 blockade. The triple regimen aims to combine the cytotoxic effects of chemotherapy with the immune-mediated anti-tumor activity from checkpoint inhibition. This approach may be under investigation for advanced or metastatic gastrointestinal cancers.
