Capecitabine + ganetespib is an investigational combination therapy evaluated primarily in oncology. Capecitabine is an oral prodrug of 5-fluorouracil (5-FU), a fluoropyrimidine antimetabolite that interferes with DNA synthesis by inhibiting thymidylate synthase and incorporating into RNA and DNA, leading to apoptosis in rapidly dividing tumor cells[2][4]. Ganetespib is a small molecule inhibitor of heat shock protein 90 (HSP90), a molecular chaperone essential for the stability and function of multiple oncogenic client proteins involved in cell growth, survival, and metastasis[6]. By inhibiting HSP90, ganetespib promotes degradation of these client proteins and disrupts key signaling pathways such as PI3K/Akt/NF-κB, Raf/MEK/ERK, and JAK/STAT3[6]. Preclinical studies have shown that combining capecitabine with ganetespib enhances antitumor efficacy compared to either agent alone in colorectal cancer models[3], likely due to complementary mechanisms—capecitabine targeting nucleic acid metabolism while ganetespib destabilizes multiple oncogenic drivers. This combination has been studied clinically for rectal adenocarcinoma alongside radiation therapy[5].
