Overview

Capecitabine + neratinib is an oral combination regimen used primarily for the treatment of advanced or metastatic HER2-positive breast cancer, particularly in patients who have received two or more prior anti-HER2-based regimens. Capecitabine is a prodrug of 5-fluorouracil, functioning as an antimetabolite that interferes with DNA synthesis. Neratinib is a potent, irreversible pan-HER tyrosine kinase inhibitor targeting human epidermal growth factor receptor 2 (HER2), HER4, and epidermal growth factor receptor (EGFR). The combination has demonstrated improved progression-free survival compared to lapatinib plus capecitabine and shows particular benefit in patients with central nervous system metastases. The most common adverse events are gastrointestinal toxicities such as diarrhea and palmar-plantar erythrodysesthesia syndrome[1][3][4][7].
