Overview

CAPOXIRI is an investigational neoadjuvant chemotherapy combination consisting of capecitabine, oxaliplatin, and irinotecan. Developed and evaluated by Beth Israel Medical Center, this regimen is primarily indicated for patients with resectable, borderline resectable, or locally advanced pancreatic adenocarcinoma. The combination aims to enhance the efficacy of standard treatments like FOLFIRINOX by utilizing the oral fluoropyrimidine capecitabine as a substitute for infusional 5-fluorouracil. Capecitabine acts as a prodrug of 5-fluorouracil, which inhibits thymidylate synthase to disrupt DNA synthesis. Oxaliplatin is a platinum-based agent that forms intra- and inter-strand DNA cross-links, while irinotecan inhibits DNA topoisomerase I, preventing the religation of DNA strands. Together, these agents provide a multi-targeted approach to induce apoptosis and inhibit tumor growth in pancreatic cancer.
