This is a combination chemotherapy regimen consisting of capecitabine, oxaliplatin, and thalidomide. Capecitabine is an oral prodrug of 5-fluorouracil (5-FU), which inhibits DNA synthesis in tumor cells by interfering with thymidylate synthase. Oxaliplatin is a platinum-based chemotherapeutic that forms DNA crosslinks, leading to apoptosis in rapidly dividing cancer cells. Thalidomide has immunomodulatory and anti-angiogenic properties; it inhibits the production of tumor necrosis factor-alpha (TNF-α) and reduces blood vessel growth in tumors. This combination has been studied as first-line treatment for metastatic colorectal cancer (MCRC), where the addition of thalidomide to the standard XELOX regimen improved disease control rate but did not significantly extend progression-free survival[1].
