Overview

This entry refers to two alternative multi-agent chemotherapy regimens used primarily in the treatment of advanced or metastatic colorectal cancer and other gastrointestinal malignancies. The first regimen is a combination of capecitabine (an oral prodrug of 5-fluorouracil) and oxaliplatin (a platinum-based chemotherapeutic), commonly known as XELOX or CAPOX. The second regimen is a triplet combination of capecitabine, irinotecan (a topoisomerase I inhibitor), and bevacizumab (a monoclonal antibody targeting vascular endothelial growth factor A). Both regimens are designed to inhibit tumor cell proliferation through distinct but complementary mechanisms—capecitabine interferes with DNA synthesis via its conversion to 5-FU; oxaliplatin induces DNA crosslinking leading to apoptosis; irinotecan inhibits topoisomerase I causing DNA damage; and bevacizumab blocks angiogenesis by inhibiting VEGF-A. These combinations are standard options for first-line therapy in metastatic colorectal cancer and may be used in other gastrointestinal cancers depending on clinical context[1][2][3][6][8].
