Overview

This combination therapy regimen consists of the oral chemotherapy agents capecitabine and temozolomide (collectively known as CapTem) administered in conjunction with Yttrium-90 (Y-90) transarterial radioembolization (TARE). Capecitabine is a fluoropyrimidine carbamate prodrug that is converted to 5-fluorouracil, which inhibits thymidylate synthase to disrupt DNA synthesis. Temozolomide is an oral alkylating agent that methylates DNA, primarily at the O6 and N7 positions of guanine, leading to DNA damage and apoptosis. Yttrium-90 is a high-energy beta-emitting radioisotope delivered via microspheres directly into the hepatic artery, providing localized internal radiation to liver metastases. This specific protocol, identified as UPCC 04219, is being evaluated by the Abramson Cancer Center at Penn Medicine for the treatment of patients with unresectable, liver-dominant, metastatic Grade 2 or 3 neuroendocrine tumors (NETs) to improve hepatic progression-free survival.
