Overview

Capivasertib + abiraterone is a combination of two oral small-molecule drugs used in investigational cancer therapy, specifically for metastatic hormone-sensitive and castration-resistant prostate cancer characterized by PTEN deficiency. **Capivasertib** is a potent, selective pan-AKT inhibitor targeting AKT1/2/3, key kinases in the PI3K/AKT pathway, which is often activated in prostate cancer, especially in the context of PTEN loss. **Abiraterone** is an androgen biosynthesis inhibitor that blocks CYP17A1, thereby reducing androgen production and signaling through the androgen receptor. This dual blockade targets two reciprocally regulated survival pathways in prostate cancer cells: androgen receptor signaling and the PI3K/AKT pathway. The combination aims to overcome therapy resistance mediated by pathway cross-talk. Clinical trials indicate the combination has acceptable tolerability and pharmacokinetics consistent with single-agent use, and trials are ongoing for efficacy in PTEN-deficient, metastatic hormone-sensitive prostate cancer[1][3][4].
