CAPOX + modified FOLFOX-7 is a combination chemotherapy strategy used in the treatment of locally advanced rectal cancer, particularly within Total Neoadjuvant Therapy (TNT) protocols. CAPOX consists of capecitabine (an oral prodrug of 5-fluorouracil) and oxaliplatin (a platinum-based antineoplastic agent). Modified FOLFOX-7 is a variation of the FOLFOX regimen comprising oxaliplatin, leucovorin (folinic acid), and 5-fluorouracil (5-FU), typically characterized by the omission of the 5-FU bolus and simplified dosing to reduce toxicity while maintaining efficacy. In the context of rectal cancer, these regimens are used as neoadjuvant or adjuvant systemic therapy to eradicate micrometastases and improve pathological complete response rates when combined with radiation therapy and surgery.
