CAR-T + CTL + DCvac is an investigational combination immunotherapy developed by the Shenzhen Geno-Immune Medical Institute for the treatment of T-cell acute lymphoblastic leukemia (T-ALL). This personalized approach utilizes the patient's own immune cells, which are modified and expanded ex vivo. The therapy consists of three primary components: multi-antigen-targeted chimeric antigen receptor T cells (CAR-T) for direct tumor recognition, antigen-specific cytotoxic T lymphocytes (CTLs) to provide sustained anti-tumor activity, and a dendritic cell vaccine (DCvac) modified with GM-CSF and B7-2 (CD86) to enhance T-cell activation and persistence. The CAR-T cells are constructed using the proprietary 4SCAR platform, which incorporates a lentiviral vector and an inducible Caspase-9 safety mechanism to mitigate potential toxicities. The treatment regimen is currently being evaluated in Phase 1 clinical trials (notably NCT04085159) with the goal of achieving next-generation sequencing minimal residual disease (NGS-MRD) negativity.
