CAR T cells targeting LBR + CAR T cells targeting TMPO is an experimental cell therapy approach designed to treat MYC-driven Group 3 medulloblastoma (MB). This therapy targets two nuclear envelope (NE) proteins, Lamin B Receptor (LBR) and Thymopoietin (TMPO), which are typically sequestered within the nucleus but are aberrantly mislocalized to the plasma membrane specifically in these aggressive pediatric brain tumors. The therapy utilizes single-domain antibodies (sdAbs) that recognize the N-termini of LBR and TMPO, allowing the engineered T cells to selectively identify and eliminate cancer cells while sparing normal neural stem cells and tissues. Preclinical studies in patient-derived xenograft (PDX) models have demonstrated potent anti-tumor efficacy, suggesting a novel therapeutic vulnerability in treatment-refractory childhood medulloblastoma. The approach includes both monospecific CAR T cell combinations and bispecific CAR T cell constructs.
