Carbamazepine + lithium is a combination of two small molecule drugs used primarily as mood stabilizers in the management of bipolar disorder, particularly for patients with rapid-cycling forms or those who are refractory to monotherapy. Carbamazepine acts mainly by inhibiting voltage-gated sodium channels, thereby stabilizing hyperexcited nerve membranes and reducing synaptic transmission. Lithium's mechanism is multifactorial but includes modulation of neurotransmitter release and second messenger systems, such as inhibition of inositol monophosphatase and glycogen synthase kinase-3 (GSK-3). The combination may provide synergistic effects for mood stabilization but carries a risk of neurotoxicity even at therapeutic plasma levels.
