Overview

This is a combination of three antiepileptic drugs (AEDs): carbamazepine, phenytoin, and valproic acid. Each drug has a distinct mechanism of action but all are used primarily in the management of epilepsy and seizure disorders. Carbamazepine acts mainly by inhibiting voltage-gated sodium channels, stabilizing hyperexcited nerve membranes and reducing synaptic propagation of excitatory impulses. Phenytoin also acts as a sodium channel blocker, prolonging the inactivated state of these channels to limit repetitive firing. Valproic acid has multiple mechanisms including increasing brain concentrations of gamma-aminobutyric acid (GABA) by inhibiting GABA transaminase, blocking voltage-gated sodium channels, and modulating T-type calcium channels. This combination may be considered in refractory or complex cases where monotherapy or dual therapy is insufficient; however, it carries significant risk for pharmacokinetic interactions and adverse effects due to overlapping metabolic pathways and protein binding displacement[1][4][5]. The use of such polytherapy should be closely monitored.
