Overview

Carbenicillin + gentamicin is a combination of two antibiotics used primarily for the treatment of infections caused by gram-negative bacteria, especially Pseudomonas aeruginosa. Carbenicillin is a broad-spectrum penicillin (a beta-lactam antibiotic) that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins, leading to cell lysis and death. Gentamicin is an aminoglycoside antibiotic that binds to the 30S subunit of bacterial ribosomes, inhibiting protein synthesis and causing misreading of mRNA. The combination exhibits synergistic bactericidal activity against certain strains of Pseudomonas aeruginosa and other gram-negative organisms; this synergy allows for enhanced efficacy compared to either drug alone at low doses[1][3][4][6][9]. However, physical mixing in vitro can lead to inactivation of gentamicin by carbenicillin under some conditions[5]. This combination has been studied both in vitro and in vivo (animal models), showing improved outcomes over monotherapy for severe infections.
