carbidopa + levodopa + naproxen is an investigational drug combination being evaluated by Northwestern University for its potential to mitigate brain functional adaptations and withdrawal symptoms in patients with chronic back pain and opioid use or misuse. This combination, often referred to in clinical studies as DA+NSAID, consists of the dopamine precursor levodopa, the DOPA decarboxylase inhibitor carbidopa, and the non-selective non-steroidal anti-inflammatory drug (NSAID) naproxen. The therapeutic rationale is to address the dysregulation of the brain's reward and dopaminergic systems caused by chronic opioid exposure while simultaneously managing pain through COX-1 and COX-2 inhibition. By providing a dopaminergic re-exposure without the use of opioids, the treatment aims to alleviate withdrawal-related brain changes and cognitive or emotional distress.
