Overview

Carbogen + phenylephrine is a combination therapy consisting of carbogen (a gas mixture of 5% carbon dioxide and 95% oxygen) and phenylephrine (an alpha-1 adrenergic receptor agonist). Carbogen acts as a cerebral vasodilator by increasing arterial CO2, which dilates cerebral arterioles and enhances cerebral blood flow. It has been used in the treatment of sudden sensorineural hearing loss, ocular ischemia, and is being investigated for progressive lacunar infarction due to its ability to improve marginal perfusion in small vessel strokes[1][2]. Phenylephrine is a selective alpha-1 adrenergic receptor agonist that causes peripheral vasoconstriction, leading to increased systemic blood pressure with minimal direct effect on cerebral arteries. It is commonly used as a decongestant or vasopressor but may also be considered for stroke management where induced hypertension can augment cerebral perfusion[2][3][5]. The combination aims to leverage both increased systemic pressure (phenylephrine) and local vasodilation (carbogen) to optimize brain perfusion in conditions like progressive lacunar infarction.
