This is a combination regimen consisting of four agents: - **Carboplatin** is a platinum-based chemotherapy that causes DNA crosslinking and inhibits DNA synthesis, leading to cell death. - **Paclitaxel** is a microtubule inhibitor that stabilizes microtubules and prevents their disassembly, thereby inhibiting cell division. - **Cetuximab** is a chimeric monoclonal antibody targeting the epidermal growth factor receptor (EGFR), blocking ligand binding and downstream signaling involved in tumor growth. - **Cixutumumab** is a fully human monoclonal antibody targeting the insulin-like growth factor 1 receptor (IGF-1R), inhibiting IGF-mediated signaling pathways involved in cancer cell proliferation and survival. This multi-agent regimen combines cytotoxic chemotherapy with targeted therapies against EGFR and IGF-1R. It has been investigated primarily for advanced solid tumors, including head and neck cancers. The rationale for combining these agents lies in attacking cancer cells through multiple mechanisms—DNA damage, mitotic arrest, EGFR inhibition, and IGF pathway blockade—to enhance antitumor efficacy.
