This drug is a combination therapy consisting of three agents: carboplatin, dulanermin, and paclitaxel. Carboplatin and paclitaxel are well-established chemotherapeutic agents widely used in the treatment of various solid tumors including non-small cell lung cancer (NSCLC), ovarian cancer, breast cancer, cervical cancer, endometrial cancer, thymoma or thymic carcinoma, and carcinoma of unknown primary. Carboplatin is a platinum-based alkylating agent that causes DNA crosslinking leading to apoptosis. Paclitaxel stabilizes microtubules and inhibits their disassembly which disrupts mitosis resulting in cell death. Dulanermin is a recombinant human Apo2L/TRAIL (tumor necrosis factor-related apoptosis-inducing ligand) that induces apoptosis by binding to death receptors on tumor cells. It acts as a pro-apoptotic cytokine mimetic targeting TRAIL receptors to trigger programmed cell death selectively in tumor cells. The combination has been studied primarily for advanced or recurrent NSCLC as first-line treatment. Clinical trials have shown that adding dulanermin to the carboplatin plus paclitaxel regimen was generally well tolerated with no dose-limiting toxicities observed; however, it did not significantly improve objective response rates compared with carboplatin plus paclitaxel alone in unselected patients. The combination demonstrated antitumor activity with an overall response rate around 38-58% depending on study cohorts and dosing schedules.
