Overview

This is a five-drug combination regimen consisting of carboplatin, epirubicin, paclitaxel, pertuzumab, and trastuzumab. It is used as neoadjuvant or adjuvant therapy in patients with HER2-positive breast cancer. The regimen combines two cytotoxic chemotherapies (carboplatin and paclitaxel), an anthracycline (epirubicin), and dual HER2-targeted monoclonal antibodies (pertuzumab and trastuzumab). Carboplatin induces DNA crosslinking leading to apoptosis; epirubicin intercalates into DNA inhibiting topoisomerase II; paclitaxel stabilizes microtubules preventing cell division; both pertuzumab and trastuzumab bind to different epitopes on the HER2 receptor, blocking downstream signaling pathways critical for tumor growth. This combination aims to maximize pathologic complete response rates while balancing toxicity risks such as cardiotoxicity from anthracyclines[1][4][6][8].
