Carboplatin + etoposide + ifosfamide is a combination chemotherapy regimen utilized in the SIOP CNS GCT II clinical trial for the treatment of intracranial germ cell tumors (GCTs) in children and young adults. This regimen, often administered in alternating cycles (e.g., etoposide/carboplatin alternating with etoposide/ifosfamide), is designed to provide risk-adapted therapy to maintain high survival rates while minimizing long-term toxicity from radiation. Carboplatin and ifosfamide are alkylating agents that interfere with DNA replication by forming covalent bonds and cross-links, whereas etoposide is a semisynthetic derivative of podophyllotoxin that inhibits topoisomerase II, preventing DNA ligation and inducing cell cycle arrest. The trial, led by University Hospital Muenster, aims to optimize outcomes for both germinoma and non-germinomatous germ cell tumors through stratified chemotherapy and reduced-field radiotherapy.
