Modified Triple Doublet Therapy is a sequential chemotherapy regimen developed by Massachusetts General Hospital for the treatment of newly diagnosed advanced malignancies of Müllerian origin, including ovarian, fallopian tube, and primary peritoneal cancers. The protocol consists of nine cycles of treatment divided into three distinct sequential doublets: three cycles of carboplatin and gemcitabine, followed by three cycles of carboplatin and paclitaxel, and concluding with three cycles of doxorubicin and topotecan. This approach utilizes five cytotoxic agents with diverse mechanisms of action—DNA alkylation (carboplatin), antimetabolite activity (gemcitabine), microtubule stabilization (paclitaxel), DNA intercalation and topoisomerase II inhibition (doxorubicin), and topoisomerase I inhibition (topotecan)—to maximize tumor cell kill and overcome potential resistance in advanced disease. A Phase II trial demonstrated that the regimen could be delivered with acceptable toxicity and showed encouraging pathologic complete response rates.
