Overview

This is a combination chemotherapy regimen consisting of four cytotoxic agents—carboplatin, gemcitabine, paclitaxel, and pemetrexed. Each drug has a distinct mechanism of action targeting cancer cell proliferation and survival. Carboplatin is a platinum-based compound that induces DNA crosslinking and apoptosis. Gemcitabine is a nucleoside analog that inhibits DNA synthesis by incorporating into DNA strands during replication. Paclitaxel stabilizes microtubules, preventing their depolymerization and thereby inhibiting mitosis. Pemetrexed is an antifolate agent that disrupts folate-dependent metabolic processes essential for cell replication by inhibiting multiple enzymes involved in nucleotide synthesis. This combination has not been widely reported as a standard regimen; however, each component (and various doublet or triplet combinations) are established treatments for advanced non-small cell lung cancer (NSCLC) and other solid tumors[2][4][5]. The rationale for combining these agents would be to maximize antitumor efficacy through complementary mechanisms of action.
