This is a combination regimen consisting of three drugs: carboplatin, icotinib, and pemetrexed. - **Carboplatin** is a platinum-based chemotherapeutic agent that causes DNA crosslinking and inhibits DNA synthesis, leading to apoptosis in rapidly dividing cells. - **Icotinib** is an orally administered, reversible small-molecule epidermal growth factor receptor (EGFR) tyrosine kinase inhibitor (TKI), primarily developed and used in China for the treatment of non-small cell lung cancer (NSCLC) with EGFR mutations[9]. - **Pemetrexed** is a folate analog metabolic inhibitor that disrupts folate-dependent metabolic processes essential for cell replication by inhibiting thymidylate synthase, dihydrofolate reductase, and glycinamide ribonucleotide formyltransferase[7][10]. The combination has been studied as first-line or maintenance therapy in advanced non-squamous NSCLC patients—especially those harboring EGFR mutations or wild-type EGFR—where it may be given as induction chemotherapy followed by maintenance therapy with either two or all three agents[1][2][3][4][6]. The rationale for combining these agents lies in their complementary mechanisms: cytotoxic chemotherapy (carboplatin/pemetrexed) targets rapidly dividing tumor cells while the targeted agent (icotinib) inhibits oncogenic signaling through EGFR.
